Ensure you have these items ready for your appointment. The NHS App is the most preferred method for providing and accessing your medical records, so please bring your records through the NHS App if possible.

How to request Your Medical Records?

Contact your GP surgery and request a copy of your Full medical records from birth.

Provide your details (full name, date of birth, address, and NHS number if known).

Wait up to 28 days for your GP surgery to process the request.

Understanding the D4 Form

The D4 Medical Examination Report is the standard document required by the DVLA. It must be completed by a GMC-registered doctor and a qualified optician (though our doctors can often perform the vision assessment).

Your Responsibility

You must complete the right side of the first page, sign the bottom of each page, and the left side of the final page.

Medical Validity Timeline

Under 45 Years Old

Your initial medical is required when you first apply for your provisional HGV/PCV license. It is usually valid for 5 years.

Aged 45 to 65

From the age of 45, you must undergo a D4 medical every 5 years to renew your professional driving entitlement.

Over 65 Years Old

Drivers aged 65 and over are required to complete a D4 medical assessment every single year.

A typical taxi medical appointment takes approximately 15 minutes if you have all your paperwork ready.

Most council require a Medical summary from the GP. NHS APP is preferred. Please note Wolverhampton and TFL require the FULL MEDICAL HISTORY ( NHS APP allows full medical acesss therefore is preferred)
