Note for Taxi Drivers

Important: Wolverhampton Taxi Licensing Council and TFL (Transport for London) require full medical records, not a summary. Please ensure you request complete medical records from your GP surgery if you are applying through these authorities. You may also need proof of address (utility bill or bank statement) dated within the last 3 months.

What to Bring with you Checklist

Vision Needs

Bring your most recent glasses prescription and any contact lenses used for driving. We'll need to check your vision with and without correction.

Medical Records

NHS APP is the preferred option. However you can also bring Medical summary from GP as well as clinic letters.

Valid Photo ID

Valid Photo ID is essential - bring your Driving License or Passport. We cannot proceed without verifying your identity.

How to request Your Medical Records?

Contact your GP surgery and request a copy of your Full medical records from birth.

Provide your details (full name, date of birth, address, and NHS number if known).
