Essential Checklist
- The relevant medical form (D4 or Council-specific Taxi form)
- Photo ID (Driving License or Passport)
- Glasses or contact lenses and your latest prescription
- Details of any medical conditions and consultant reports
- List of all current medications
Note for Taxi Drivers
NHS APP is the preferred option. However you can also bring Medical summary from GP as well as clinic letters.
Important: Wolverhampton Taxi Licensing Council and TFL (Transport for London) require full medical records, not a summary. Please ensure you request complete medical records from your GP surgery if you are applying through these authorities. You may also need proof of address (utility bill or bank statement) dated within the last 3 months.
